Weight loss after sleeve gastrectomy is due to increased circulating bile acids that bind to the nuclear receptor (Farsenoid-X receptor - FXR) and alteration in gut bacteria. It is not due to mechanical restriction. If FXR is absent then weight loss and diabetes remission will be less.
