SLEEVE vs BYPASS vs GHRELIN



After 'gastric bypass' fasting ghrelin (appetite stimulating hormone) is normal. But there is enhanced ghrelin suppression after food intake.
After 'sleeve gastrectomy' fasting ghrelin is four fold reduced. There is post-prandial ghrelin suppression but less than after gastric bypass.

Meta analysis of international bariatric surgery studies from 2003 to 2012 (Published in JAMA Surg, Dec 18, 2013) provides reassurance that bariatric surgery is an effective treatment method for permanent weight loss in obese individuals. It has very low mortality and acceptable risk of morbidity. This study also confirms that bariatric surgery improves many of the obesity related co-morbid conditions such as type 2 diabetes, hypertension, and sleep apnea.

Gastric bypass (RYGB) - More effective weight loss & More complications
Sleeve gastrectomy (SG) - Similar weight loss effects as gastric bypass
Gastric banding (LAGB) - Less effective weight loss & High reoperation rate
Leak rate - Most feared complication & is < 2%
